What I used to do (before I started using tutelage) is do a hideout with just MC + companion you want to train. You tank all the dudes, they kill them with the chosen weapon. then go up and murder looters 3-5 versus 100. get yourself/companions 100 levels of bow and riding in ONE battle. take them up and go within 130ish range. stop. let your companions fire while not moving. as looters get within 50m, move again. go back out to 130ish. stop again. repeat. stopping lets your companions aim better, and get his.
With this method it makes it way less of a grind to level up medicine, since you care a lot less about losing high-tier troops. Put em through the meat grinder and when you're low, go grab a bunch of bandits and/or regular recruits, donate a couple 2H swords, and bam, you're back to a full stack of max-tier troops.
So far it seems it's one of the hardest skills to level up companions with (haven't bothered doing it myself), along with roguery and trade. I do know trade is easier for the MC tho, but it also seems to crawl for NPCs. Like I have been having a companion run a caravan for years and her trade hasn't increased enough to get a single extra perk.
I've been fighting with my 80 stack + my companion's 50 stack and just realized i dont know if the troops in my companions party are leveling up or not. I do know that there are some low level companions available at some point, not always at the game start though. Some companions get generated over time or when you reach a new clan tier.
